Physicals

Physicals:

Each swimmer needs to have a sports physical completed by the first day of practice.

Log on to: Athletic Clearance

At this website, you can register your student athlete & download the necessary forms for their sports physical.

Once the student athlete has completed their physical, please turn in paperwork to SLC B. This is where they will be cleared to start practice!
